True Health NY is seeking a dependable, clinically strong and patient-centered Primary Care Physician to serve as the physician anchor for our Fresh Meadows clinic in Queens.

We welcome physicians trained in either Internal Medicine or Family Medicine who are committed to comprehensive adult outpatient care. The successful physician will build and maintain a stable patient panel, expand access to primary care, manage acute and chronic medical conditions and provide consistent clinical leadership at the Fresh Meadows location.

The Fresh Meadows clinic offers an integrated environment with adult primary care, OB/GYN, women’s health, ultrasound and coordinated diagnostic and administrative support.

This is a site-anchored position—not a four-office float role. The physician will have a defined Fresh Meadows schedule, dedicated support and clear responsibility for patient continuity and clinical follow-through.

Clinical Responsibilities

The Primary Care Physician will:

  • Provide comprehensive adult outpatient primary care.

  • Conduct new-patient examinations, annual physicals, preventive visits and follow-up care.

  • Provide Medicare annual wellness visits when appropriate.

  • Offer same-day and acute-care appointments when clinically appropriate.

  • Diagnose and manage common acute and chronic medical conditions.

  • Manage hypertension, diabetes, hyperlipidemia, obesity, thyroid disease, asthma, COPD, cardiovascular risk factors and other common adult health needs.

  • Provide medication management and medication reconciliation.

  • Conduct preoperative medical evaluations and appropriate risk assessment within scope.

  • Provide evidence-based preventive screening and immunization counseling.

  • Order and interpret laboratory, imaging and diagnostic testing.

  • Review and communicate clinically significant findings promptly.

  • Develop clear treatment and follow-up plans.

  • Coordinate referrals and consultation with specialists.

  • Maintain continuity for medically complex and high-risk patients.

  • Identify conditions requiring emergency, hospital or specialty escalation.

  • Provide telehealth services when clinically appropriate and operationally approved.

  • Participate in population-health, preventive-care and chronic-disease initiatives.

  • Support appropriate PCP assignment and panel development in accordance with payer requirements.

Integrated Care Responsibilities

The physician will work collaboratively with the clinic’s OB/GYN and women’s-health clinicians to:

  • Coordinate primary care for patients receiving gynecologic or prenatal services.

  • Address chronic medical conditions that affect reproductive and women’s health.

  • Provide appropriate preconception medical evaluation and chronic-disease optimization.

  • Coordinate postpartum transition back to comprehensive primary care.

  • Support preventive-care and screening completion.

  • Avoid unnecessary duplication of testing and fragmented referrals.

  • Maintain appropriate clinical boundaries and referral pathways between specialties.

The Primary Care Physician will remain responsible only for services within the physician’s training, licensure, scope and assigned role.

Documentation and Patient Follow-Through

The physician will be expected to:

  • Complete encounter documentation accurately and within established practice timeframes.

  • Enter required orders and charge-support information promptly.

  • Review and act on laboratory, imaging and diagnostic results.

  • Communicate clinically significant results to patients clearly and promptly.

  • Respond appropriately to patient messages, refill requests and clinical escalations.

  • Document referrals, follow-up instructions and care plans clearly.

  • Maintain an organized clinical inbox without a persistent unresolved backlog.

  • Ensure appropriate handoff and cross-coverage when unavailable.

  • Comply with clinical, regulatory, payer and organizational policies.

Clinical Leadership and Collaboration

The physician will:

  • Collaborate with nurse practitioners, OB/GYN physicians, women’s-health clinicians, medical assistants, sonographers and administrative staff.

  • Provide appropriate clinical consultation to advanced-practice clinicians and staff within applicable law, credentialing and organizational policy.

  • Promote respectful and effective teamwork.

  • Participate in peer review, quality improvement, patient-safety and clinical-policy initiatives.

  • Help develop efficient workflows that improve access without compromising clinical quality.

  • Support a culture of accountability, responsiveness and patient-centered care.

  • Represent True Health NY professionally in the Queens community.

Performance Expectations

True Health NY is seeking a physician who demonstrates:

Patient-volume expectations will be introduced through a structured onboarding ramp and adjusted for patient complexity, new-patient volume, procedures, quality and schedule design.

The position is not based on an inflexible visit count regardless of clinical circumstances.

Required Qualifications
  • Doctor of Medicine or Doctor of Osteopathic Medicine degree

  • Completion of an accredited Internal Medicine or Family Medicine residency

  • Board certified or board eligible in Internal Medicine or Family Medicine

  • Active and unrestricted New York medical license, or clear eligibility to obtain one

  • Current New York registration before beginning independent practice

  • Active DEA registration, or eligibility to obtain one

  • Current NPI and eligibility for Medicare, Medicaid and commercial-payer enrollment

  • Eligibility for professional malpractice coverage

  • Eligibility to participate in federal and state healthcare programs

  • Strong outpatient clinical judgment

  • Strong communication and documentation skills

  • Ability to work consistently at the Fresh Meadows location

Preferred Qualifications
  • Two or more years of outpatient primary-care experience

  • Experience developing, inheriting or managing a longitudinal patient panel

  • Experience managing medically complex adult patients

  • Experience with Medicare, Medicaid and diverse community populations

  • Experience with eClinicalWorks or a comparable electronic medical record

  • Experience in a community-based or multisite medical practice

  • Interest in quality improvement, population health or physician leadership

  • Additional language skills relevant to the Queens patient population

Highly qualified graduating residents and early-career physicians may be considered when they demonstrate strong clinical references, appropriate training and a clear commitment to outpatient primary care.

Compensation

True Health NY expects to offer an annual base salary of $280,000–$320,000 for this position.

The final base salary will depend on factors including:

  • Board-certification status

  • Relevant outpatient experience

  • Clinical background and scope

  • Schedule

  • Credentialing readiness

  • Professional references

  • Internal equity

  • Other lawful considerations

The physician will also be eligible to participate in a written productivity and quality incentive plan.

Incentive compensation is separate from the stated base-salary range and will be explained before an offer is accepted.

A sign-on bonus may be available for a qualified candidate and, when provided, will be subject to a written service and repayment agreement.
