Job Description

Housing and Health Care Stabilization Coordinator

JobSummary: TheHousing and Health Care Stabilization Coordinator will lead and develop theprocesses of the Hospital to Home program to connect people with serious andpersistent substance use from the hospital to home. The coordinator will createa system to provide holistic care for unsheltered people with physical andbehavioral health needs. This position will provide direct administrative,social services, and programmatic services support for the Residential Teamsand Hospital staff who are part of the Estadt Lubert Collaborative. Thecoordinator will be a liaison between Project HOME, health systems, and thehousing resources for referred participants by connecting them to resources andprogramming. This will involve meeting with the referred participants toevaluate the appropriate resources and programming available.

Administrative skilled servicesinclude, but are not limited to, compliance with all policies and procedures,as well as reporting matrixes for compliance with funders (which directlyimpacts funding); operational directives to maintain a safe and healthyenvironment; ensures intakes and assessments and data entry into the HMISsystem within 24 hours; maintains of accuracy all required documentation; andother duties as assigned.

Programmaticand Social Services support include, but are not limited to, seamlesscoordination and integration of residential and medical services; and utilizessuch information to inform Collaborative to refine and enhance programming. Leads the Housing and Clinical Care Committeeof the Collaborative. Develops trainingfor key staff and partners and executes informational sharing among residentialsites and healthcare partners. Activelyengage in a team-based model of service delivery, whereby the Housing andHealth Care Stabilization Coordinator will be proactive in seeking additionalexternal and internal resources to meet the needs of the individual servedthrough the respective residential and health care teams.

EssentialDuties and Responsibilities

  • Developing and monitoringeffective workflows of hospitals and residential sites to ensure continuity ofcare.
  • Coordinationof incoming participants from the health systems to the appropriate housingresources and programming.
  • Conductmeetings and/or visits with participants once placed.
  • Manages the integrationof health care services in collaboration with the residential sites to addressthe needs of the residents participating in the Hospital to Home program.
  • Activemanagement and partnership with the health systems and housing resources toconnect participants to appropriate resources and programming.
  • Assesses andcommunicates to hospitals and residential sites any challenges or workflowimprovements.
  • Manages intake andprovides daily census to hospitals of their beds.
  • Ensure Policies andProcedures are operationalized and adhered to.
  • Create Key Performance Indicatorsthat align with the vision and outcomes of the Estadt Lubert Collaborative.
  • Ensure up-to-date information on all affordableand appropriate housing resources for the Estadt Lubert Collaborative.
  • Understands andappreciates the demands of the population served and works to support atrauma-informed environment.
  • Responsible foroverseeing the HMIS reporting at all residential sites.
  • Collaborate withcolleagues and develop recommendations to present to the Director to add (ordecrease) services based upon individual's served needs.
  • Special projects and other duties, as assigned.
  • Be a champion of ProjectHOME's commitment to diversity, equity, and inclusion.
  • Represent Project HOMEin meetings related to the Estadt Lubert Collaborative.

Education and Experience Requirements:

  • Registered Nurse with a Bachelor of Science in Nursing (BSN) (preferred).
  • Registered Nurse with an Associate of Science in Nursing (ASN).
  • Licensed Social Worker (LSW) with experience in a healthcare setting with people using substances and/or are dually diagnosed.
  • Master's degree in behavioral health or related field of study.
  • At least 3 years of workexperience with people using substances and/or are dually diagnosed.

  • Ability to work in afast-paced environment with rapid turnaround times.
  • Ability to work in afluid structural environment requiring a high degree of collaboration.
  • Strong analytical and numerical skills.
  • Knowledge of advanced Word, Excel, Access, PowerPoint,and other highly complex computer skills.

Work Schedule: 9:30 AM to 5:30 PM Monday thru Friday,Evenings/weekends may be necessary periodically.

Physical Requirements: Sedentary, some walking
