Job Description

Pharmacy Clinical Coordinator

The Pharmacy Clinical Coordinator will be responsible for providing clinical and dispensing pharmacy services to patients receiving care at Randolph Hospital. This position will assume additional responsibilities related to evaluation, development, and implementation of pharmacy services and medication management practices. During the performance of these responsibilities, the Pharmacy Clinical Coordinator will work closely with the Director of Pharmacy, members of the Pharmacy Department, the Pharmacy and Therapeutics Committee, Nursing Staff and Administration, Quality Management Staff, Senior Administration, and the Medical Staff of Randolph Hospital. Responsibilities will include: collection and assessment of data related to medication use practices, adverse drug events, medication errors, and clinical pharmacy services. Development of evidence-based recommendations and presentation of this information to appropriate hospital and medical staff. Evaluation and development of policies, procedures, and protocols related to medication safety and provision of clinical pharmacy services. Development of educational programs and medication therapy resources for physicians, nurses, pharmacists, and other health care providers. Coordination of professional experiential programs for students of North Carolina-based Schools of Pharmacy. This position will serve as a voting member of the Pharmacy and Therapeutics Committee. Responsibilities to that committee may include preparation and presentation of information for formulary decisions and other discussion topics, assistance with agenda preparation and documentation of meeting minutes, and dissemination of committee decisions and information to other hospital staff. The Pharmacy Clinical Coordinator will participate in other multidisciplinary committees, teams, and projects as assigned.

Education/Degrees(s): The successful candidate must hold a Doctor of Pharmacy degree from an accredited School of Pharmacy and have either a general acute care Pharmacy Practice residency or a minimum of three years of hospital pharmacy experience.

License(s): The successful candidate must hold and maintain a current license to practice pharmacy in the state of North Carolina

Continuing Education: The Pharmacy Clinical Coordinator must obtain at least fifteen hours of continuing education annually from an approved provider and must complete all mandatory Randolph Hospital continuing education programs. The Pharmacy Clinical Coordinator is encouraged to obtain additional continuing education and/or advanced certifications and is expected to maintain a current knowledge base regarding clinical pharmacy services and practice standards.
