Boulder Community Health is seeking a Performance Improvement Specialist who will support quality and performance improvement initiatives, patient safety, regulatory, and support evidence-based practices. The position will also support the development of a culture of continuous improvement within the organization and facilitate organizational performance improvement events.

The Performance Improvement Specialist leads and facilitates clinical and/or operational process improvement initiatives across the organization using clinical/process improvement methodologies, project management strategies, leadership, and change management skills to achieve outcome improvements. Collaborates with staff, leadership, and patients in multidisciplinary teams to strategize, develop and achieve goals for improving the delivery of consistently safe, effective, efficient, patient-centered, timely and equitable care.

Qualifications:
•Bachelor’s degree is required, with preference given to individuals with a degree in Hospital Administration, Business Administration, Organizational Development, Nursing or ancillary area (Medical Technologist, Pharmacist, Rad Technician).
•Formal education, specialized training, or certification in Lean, continuous improvement, change management, and/or project management, with the ability to apply these principles to organizational improvement initiatives.
•Minimum 5 years’ experience working in a healthcare setting required.
•Minimum 3 years’ experience in healthcare quality improvement required.

Responsibilities:
•Leads and facilitates organizational improvement initiatives, ensuring projects are appropriately planned, structured, implemented, and sustained to achieve desired outcomes. Coordinates stakeholder engagement, communication, implementation activities, and follow-up efforts to support successful change adoption and measurable results.
•Provides education and coaching to leaders, staff, and teams to support performance improvement initiatives, organizational change, and successful project implementation.
•Partners with leaders and teams to develop standard work, project charters, communication plans, and implementation tools that support process improvement, change adoption, and sustained performance outcomes.
•Collaborates with clinical and operational leaders to identify improvement opportunities, define project objectives, establish performance measures, and monitor progress toward desired results.
•Facilitates cross-functional collaboration and stakeholder engagement to support organizational change initiatives, improve processes, and foster a culture of continuous improvement and high performance.
•Analyzes data, monitors key performance indicators, and evaluates outcomes to assess project effectiveness, identify improvement opportunities, and guide decision-making
