Job Overview:
We are seeking a Family Navigator Lead to oversee efforts in identifying and addressing family social needs and structural barriers. In this role, you will supervise a dedicated team of Family Navigators to collectively review patient data and connect families with vital support systems. This position offers a rewarding opportunity to collaborate across hospital departments, develop impactful patient protocols, and champion family-centered care while maintaining strict confidentiality.
Qualifications:
Required:
- Excellent communication skills and strong leadership capabilities
- In-depth knowledge of performance metrics
- Must be self-motivated, highly organized, and able to work independently
- Ability to incorporate and model CSH’s core values
- Familiarity with computer software packages, particularly Microsoft Word and Excel
Preferred:
- Proven work experience as a team leader or supervisor
- Previous hospital or physician's office experience
Scheduling Requirements
- Part-Time (30 hours/week)
- Monday through Friday, 8:30AM - 3:00PM
- No weekends
Essential Functions:
- Supervise, manage, and motivate the Family Navigator team on a daily basis to ensure operational success.
- Set clear team goals, monitor daily performance, and generate comprehensive metric reports.
- Oversee the collection of data regarding family social needs, systemic barriers, and issues at Children's Specialized Hospital.
- Guide the team in reviewing patient data to identify and implement optimal ways to support families in need.
- Collaborate independently with other hospital departments to develop new protocols and administrative policies.
- Ensure strict confidentiality of all collected patient and family information across all team activities.
