Job Overview:

Assists AVP of Clinical Support with patient movement by tracking and removing any barriers to patient placement in a bed. Provides patient support to facilitate early discharge by connecting Hospital and community resources. Works to identify and implement patient experience improvement activities and ensure an excellent experience of care for patient and families in all care settings. This is accomplished through facilitating coaching and learning interventions to improve patient experience.

Qualifications:

Required:

  • BA or MA in related field
  • 1 Year Hospital Experience

Essential Functions:

  • Employee Engagement: Develop strategies to ensure that employees have a positive experience, as well as make sure that the clinical support departments are living up to its core values. Tracking division “pebbles” and overseeing the “you said…we did” program.
  • Leadership: Coordinate and manager the safe and efficient discharge of patients from hospitals to their next phase of care through the logistics department. Facilitate improvement of discharge order to exit from hospital goal.
  • Team Collaboration: Collaborate with internal teams, including admissions, clinical, logistics, scheduling and administrative staff, to help facilitate communication and coordination of patient care and administrative activities.
  • Patient Advocate: helps patients and community members navigate the healthcare syste m diagnosis to treatment and follow-l 5.
  • Patient Experience: Communicates with patients who have concerns, complaints grievances, or general feedback about their care experience to facilitate resolution within the clinical support division.
