Job Overview: As a member of the health care team, the Certified Diabetes Care and Education Specialist (CDCES) will be responsible for diabetes education, care of patients and families, including new onset and established diabetes, in children, adolescents, and young adults, in the inpatient and or outpatient settings, individually and in group format. Qualifications: Required:-
Bachelor’s degree in Nursing, Nutrition, Pharmacy, or related health field
Current certification as a Certified Diabetes Care and Education Specialist (CDCES) by the National Certification Board for Diabetes Educators (NCBDE) or equivalent credential
Registered Nurse (RN), Registered Dietitian (RD), Pharmacist (RPh), or other healthcare professional licensure as required by state law
AHA BLS Provider Certification
Minimum of 1 year of clinical experience in diabetes education and management
Minimum of 1 year of clinical experience in pediatrics
Minimum of 1 year clinical experience in diabetes education and management
Evidence of excellent verbal and written English communication skills
Ability to collaborate with others

Provide patient education and ongoing patient support in consultation with physician and advanced practice nursing staff
Perform complete educational and skills assessments for patients with Type 1 and Type 2 diabetes managed on blood glucose monitoring devices, oral medications, insulin injections and insulin pumps
Provide diabetes self-management education to patients and families including individual, group and classroom settings and electronically, telephone and web based
Assist in the development, documentation and implementation of education and treatment plans for patients as well as evaluating and meeting patient and family learning needs
Evaluate achievement of learning goals and assist patients and families with making decisions about diabetes self-care including meal planning, carbohydrate counting, planning for exercise, daily schedule adjustments, responding to high and low blood sugar readings and other questions
Assist patient and family with sick day management guidelines to identify and coordinate resources
Collaborate with nursing staff, chronic illness team, and other health care professionals in the planning, provision, and evaluation of patient and family education and implementation of system change related to diabetes care and education
Support research-based projects, policy revisions, as appropriate, and assist to maintain the American Diabetes Association certification for Children’s Specialized Hospital Program
All practice is based on the legal scope of practice, national and specialty CDE standards, and in accordance with all applicable laws and regulations
