What You’ll Do:

  • Coordinate the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments
  • Ensure compliance with Medicare, Medicaid, OBRA, and other payer program requirements
  • Ensure MDS assessments accurately reflect each resident’s physical, mental, and psychosocial status
  • Verify appropriate documentation to support services provided and reimbursement accuracy
  • Support interdisciplinary collaboration for accurate care planning, assessment completion, and ongoing case management coordination within the skilled nursing environment
  • Collaborate with clinical and administrative teams to support case management functions related to admissions, transitions of care, and payer communication as it relates to MDS and skilled nursing requirements

What You’ll Need:

  • Graduate of an accredited school of nursing with a current RN licensure in the state of employment
  • Previous work experience in a long-term care or skilled nursing setting is preferred
  • MDS 3.0 and case management experience in a skilled nursing or post-acute care environment required
  • RAC-CT certification or willingness to obtain within 6 months of hire and maintain recertification as required
  • Ability to make mature judgments, function independently, remain flexible, and work effectively with interdisciplinary personnel
  • Working knowledge and proficiency with Microsoft Office required
