- Provide compassionate, patient-centered hospice nursing care
- Develop and implement individualized plans of care in collaboration with patients, caregivers, providers, and the interdisciplinary team
- Educate patients and families about hospice philosophy, services, and care expectations
- Perform ongoing assessments and document patient progress, interventions, and outcomes accurately
- Partner closely with physicians, healthcare professionals, hospice team members, and volunteers to coordinate care
- Support patients and caregivers through meaningful education and emotional support
- Participate in interdisciplinary team meetings, quality initiatives, and agency programs
- Contribute to a culture of continuous learning and excellence in care

Requirements:- Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).
- Minimum of two years acute care nursing experience.
- Three to four years of recent general medical/surgical or skilled/acute care nursing experience.
