Clinical Therapist at Woodward Community Based Services – Urbandale, Iowa

· Provide Individual, Family Therapy, and/or group therapy mental health services

· Completion of all required documentation in accordance with state regulations and Joint Commission Standards:

Comprehensive Assessment, Active Treatment Plan and Suicide Risk assessment completed within 30 days of admit, progress

notes within 72 hours of service provision

· Embed Measurement-Based Care into treatment services with the utilization of PUPS (EHR) and Blueprint.

· Abide by The Health Insurance Portability and Accountability Act of 1996 (42 USC & 201 et seq.)

· All other duties as assigned

A master’s degree in social work or human services is required. Individual must have a license with the state of Iowa or working towards this license postgraduate.
