Human Touch Home Health is seeking a licensed Physical Therapist to provide in-home therapy services in Fredericksburg and surrounding areas. We’ve served our communities for over 27 years and offer flexible schedules, strong support, and meaningful patient care.

Responsibilities

• Perform in-home PT evaluations and treatments

• Develop and adjust individualized care plans

• Educate patients and caregivers

• Document care in EMR (WellSky or HCHB preferred)

• Collaborate with interdisciplinary team

• Supervise PTAs

• Ensure regulatory compliance

Qualifications

• Active Virginia PT license

• Accredited PT program graduate

• 3+ years experience (home health or SNF preferred)

• EMR experience; OASIS a plus

• Valid driver’s license & reliable transportation
