Job Description

Job Summary

Unified Care Hospice is seeking a compassionate and dependable Bilingual Certified Nursing Assistant (CNA) to join our growing clinical team. In this role, you will provide hands-on personal care and emotional support to patients and families in their homes and care facilities. Positions are available for Full-Time, Part-Time, and PRN (as-needed) schedules.

Bilingual (English/Spanish) candidates are strongly preferred to better serve our diverse community.

Employment Status Options

  • Full-Time: Regularly scheduled hours with benefits eligibility
  • Part-Time: Consistent, limited hours based on availability and census
  • PRN (As Needed): Flexible scheduling based on patient needs; no guaranteed hours

Responsibilities

  • Provide personal care including bathing, grooming, dressing, toileting, and feeding
  • Support patient comfort, safety, and dignity at all times
  • Monitor and document patient condition and vital signs in the EMR
  • Follow the individualized Plan of Care under RN supervision
  • Assist with safe transfers and mobility using appropriate equipment
  • Maintain infection control and safety standards
  • Communicate clearly with patients, families, nurses, and leadership
  • Complete daily/same-day charting per agency policy
  • Deliver supplies and assist with basic patient support as needed
  • Participate in required trainings and in-service education

Qualifications & Experience

  • Active CNA certification in the State of Texas
  • Current CPR certification
  • Bilingual (English/Spanish) preferred
  • Valid driver’s license, reliable transportation, and active auto insurance
  • Prior hospice, home health, or long-term care experience preferred
  • Familiarity with EMR systems
  • Strong professionalism, reliability, and time management skills
  • Ability to work independently and within an interdisciplinary team
  • Experience with dementia, behavioral health, or end-of-life care is a plus

Physical Requirements

  • Ability to lift, reposition, and assist patients safely
  • Ability to stand, walk, bend, and stoop for extended periods
  • Ability to travel to patient homes and facilities
