Job Description
Affinity Healthcare Solutions Skilled Care is looking for compassionate, professional Physical therapists to help our clients in the community. We are looking for candidates who can help clients primarily in Philadelphia and Montgomery county area. We also cover Delaware, Chester, Lehigh and Bucks county.
As a Physical Therapist, you will provide high-quality care to patients in the comfort of their own homes. You will play a crucial role in helping patients regain their mobility and independence, improve their quality of life, and promote overall well-being. Strong clinical skills, computer skills, and PT experience in Home Health Care setting is preferred.
Candidates must be:
1) Able to travel with reliable transportation as a licensed driver.
2) Licensed as a PT for more than two years from an accredited, reputable program
3) Incorporate home settings to optimize therapy outcomes, ensuring each patient feels secure.
4) Partner with healthcare clinical team, from doctors to home health nurses to ensure seamless coordination of care.
5) Educate patients and caregivers on therapeutic exercises, equipment, and preventive measures.
6) Maintain comprehensive patient records, track progress, and adjust treatments accordingly.
7) Complete patient notes and communication within 24 hours.
