We are seeking an experienced Home Health Physical Therapist to provide high-quality, patient-centered care in the home setting. This role offers autonomy, flexibility, and strong interdisciplinary support.
Responsibilities-
Perform comprehensive PT evaluations and assessments

Develop and implement individualized treatment plans

Provide skilled therapy to improve mobility, strength, balance, and function
-
Educate patients and caregivers on exercises, safety, and fall prevention
-
Monitor progress and modify plans as needed
-
Accurately document care in EMR systems
-
Collaborate with physicians and interdisciplinary teams
-
Ensure compliance with Medicare, HIPAA, and agency standards
-
Supervise Physical Therapist Assistants (PTA)
-
Graduate of an accredited Physical Therapy program
-
Current Pennsylvania PT license
-
Minimum 3 years PT experience (home health or SNF preferred)

Strong clinical judgment and communication skills

Experience with OASIS preferred

EMR experience (WellSky/Kinnser or Homecare Homebase preferred)

Ability to work independently and manage time efficiently

Valid driver’s license and reliable transportation
