We are seeking an experienced Home Health Physical Therapist to provide high-quality, patient-centered care in the home setting. This role offers autonomy, flexibility, and strong interdisciplinary support.

• Perform comprehensive PT evaluations and assessments

• Develop and implement individualized treatment plans

• Provide skilled therapy to improve mobility, strength, balance, and function

• Educate patients and caregivers on exercises, safety, and fall prevention

• Monitor progress and modify plans as needed

• Accurately document care in EMR systems

• Collaborate with physicians and interdisciplinary teams

• Ensure compliance with Medicare, HIPAA, and agency standards

• Supervise Physical Therapist Assistants (PTA)

• Graduate of an accredited Physical Therapy program

• Current Pennsylvania PT license

• Minimum 3 years PT experience (home health or SNF preferred)

• Strong clinical judgment and communication skills

• Experience with OASIS preferred

• EMR experience (WellSky/Kinnser or Homecare Homebase preferred)

• Ability to work independently and manage time efficiently

• Valid driver’s license and reliable transportation
