MDS Auditor

Citizens Memorial Hospital (CMH) is recruiting a Registered Nurse (RN) to join our LTC Nursing Administration as an MDS Auditor. CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Job Summary: The MDS Auditor Nurse provides hands-on support to long-term care facilities during staffing vacancies, transitions, or employee leave and as needed per facility needs. This role ensures accurate, timely, and compliant MDS documentation; assists with assessments; monitors key clinical qualifiers; and supports the overall quality of resident care. The MDS Auditor Nurse demonstrates full compliance with the RAI Manual and develops resident care plans in alignment with Foundation policies, Federal and State regulations, and established quality-of-care standards. This position upholds the mission, values, and objectives of CMH through professional conduct, collaboration, and reliable performance. Additional duties may be assigned to support optimal resident outcomes and organizational excellence.

A typical day includes: promoting and maintaining high-quality and medically necessary care through the analysis, review, and evaluation of clinical practices and medical records.

  • Provide hands-on MDS support to facilities during staff vacancies, onboarding periods, or employee leave.
  • Deliver direct training and coaching to nursing staff on accurate GG Section documentation.
  • Conduct routine Medicaid and Medicare 5day assessment audits to ensure regulatory compliance and documentation accuracy.
  • Review hospital return cases to identify missed assessment opportunities and ensure appropriate follow-up.
  • Monitor reports for clinical qualifiers such as oxygen use, fever, antibiotics, shortness of breath, and other key indicators.
  • Oversee monitoring, identification, and staff education related to IPA opportunities.
  • Perform monthly certification letter audits to ensure accuracy and compliance.
  • Ensure accuracy and completeness of CAA documentation, care plans, and Medicaid IDT records.
  • Collaborate with facility leadership and clinical teams to strengthen documentation practices and compliance processes.
  • Attend MDS Best Practice meetings and other required meetings as needed.
  • Maintain strong working knowledge of Quality Measures, VBP, CMI,5 Star Report, and PDPM for reimbursement and quality reporting.
  • Perform additional duties as assigned by the MDS Quality Reporting Coordinator or direct supervisor.
  • Line Listing reports quarterly
  • ADR audits for Medicare and Medicaid
  • Review quarterly CMI reports

This position is: Full-time, days

Job Requirements:

  • Education: Bachelors degree preferred, RAI Training preferred and Medicare Start To Finish training preferred
  • Required Experience: Lead MDS Role, 2 years experience in LTC/Skilled nursing providing direct resident care and/or supervision of staff, 2 years experience in MDSSkills: Attention to detail, communication skills, BLS
  • License: RN
  • Certifications: RAC Certification preferred
