Per Diem Position
Work Location: On the road
Job Summary: Provide services that help restore function, improve mobility, relieve pain and prevent permanent physical disabilities of patients suffering from injuries or disease. Restore, maintain, and promote overall fitness and health.
Essential Functions:
1. Providing initial and ongoing assessment to determine level of functioning, including OASIS Assessment at appropriate time points.
2. Responsible in gathering background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient's physical and psychological functioning.
3. Responsible in developing and revising the plan of care in consultation with the physician and other care-team members.
4. Responsible for planning, evaluation, and treatment implementation of assigned caseload.
5. Responsible in preparing clinical progress notes.
6. Responsible in supervising therapy assistants and home health aides as appropriate.
7. Responsible in evaluating outcomes.
8. Responsible in educating patients and/or families about proper safe use and recommend methods for installation of adaptive equipment.
9. Adhere to all Company policies and procedures.
10. Demonstrates flexible and efficient time management techniques.
11. Prioritize workload so as to meet productivity standards and applicable deadlines.
12. Establish and maintain positive interpersonal relations with internal and external customers.
13. Maintain a positive and respectful attitude and demeanor.
Knowledge, Skill and Experience:
1. Minimum Education (or substitute experience) required: Currently licensed by the California Board of Physical Therapy.
2. Minimum Experience required: Minimum of one year in the acute care setting. Two years experience in Home Health preferred.
3. Skills Required: Must be able to maintain strict confidentiality with regard to protected and sensitive information. Must be able to solve problems independently and confidently by applying analytical and logical thinking. Good sense and practice of proper body mechanics.
