About This Position

Description:

Under the direct supervision of the SVP of Behavioral Health, the Behavioral Health Provider (BHP) is responsible for providing trauma-informed behavioral health services at the Zufall Health Center sites. Zufall Health follows the Primary Care Behavioral Health model of integrated care. The BHP provides direct services to patients, families, and groups. They provide brief behavioral health intervention to improve patient health outcomes as well as individual and family counseling, and engages in crisis intervention. The BHP contributes to the patient care team through huddles, case consultation, patient advocacy, mental health education, and collaborative team meetings with clinical and support staff. They identify opportunities for enhancing integrated Behavioral Health Services in the primary care setting and serve as a resource to the rest of the clinical staff. BHP’s working towards clinical licensure will participate in weekly clinical supervision (individual or group) with an LCSW who is certified to provide clinical supervision.

Essential Functions, Duties and Responsibilities

  • Work in collaboration with the primary care providers to recognize and treat mental health disorders and psychosocial problems.
  • Work with the primary care team to treat and manage patients with chronic emotional and/or physical health problems.
  • Complete a Comprehensive Assessment and Treatment Plan based on patient's history and relative to the presenting problem. Follow established protocols and methodologies.
  • Collaborate with patients in the development of treatment plans and measurable goals and outcomes.
  • Review cases regularly with clinical supervisor.
  • Maintain patient records well documented and in a timely fashion.
  • Participate in crisis intervention as response for patients dangerous to self/others/property.
  • Lead group sessions and activities, and develop curricula for specific initiative (for example, Healthy Lifestyles).
  • Conduct pre- and post-intervention assessments and patient surveys to evaluate effectiveness of programs.
  • Participate and present in care conferences, patient groups, educational programs for staff and for patients, staff meetings, staff trainings and other venues as assigned.
  • Represent Zufall Health Center at community service organization meetings, behavioral health agencies and collaborating entities.
  • Participate in outreach and community educational activities as part of the Care Team as assigned by the SVP of Behavioral Health and/or Chief Medical Officer.
  • Participate in in-service educational programs and continuing education.
  • Teach and train other health center staff and students as needed.
  • Follow established departmental policies and procedures regarding quality assurance, safety, environmental and infection control standards. Assist in the development of relevant policies and procedures as the department and services evolve.
  • Participate in medical/clinical staff meetings and general staff meetings.
  • Ensure patient confidentiality under HIPAA regulations.
  • Perform other duties as assigned.

  • Excellent customer focused interpersonal skills to interact in an effective manner with patients, families, the interdisciplinary health care team and community agencies with diverse opinions, values, and religious and cultural ideals
  • Excellent verbal and written communication skills
  • Culturally sensitive and knowledgeable of multicultural mores, beliefs, and customs
  • Excellent computer skills, experience with electronic medical records a plus
  • Ability to follow instructions, given either orally or in writing
  • Ability to work independently or with little supervision
  • Excellent organizational skills such as problem solving, decision making, priority setting, and work delegation
  • Ability to complete assignments in a timely manner
  • Ability to work as a team member
  • Ability to maintain HIPAA compliance, corporate compliance and client confidentiality
  • Learn and apply the trauma informed care principles with the scope of the position
  • Bilingual in English and Spanish preferred
Requirements:

Education, Training and Experience

  • Master's Degree in Social Work, Doctorate Degree in Psychology, or Master’s Degree in Counseling
  • Licensed in New Jersey as an LSW or LAC
  • Have current BLS/CPR certification or willing to receive it
  • Minimum of two years post graduate experience in clinical/ambulatory care setting
  • Experience in working with groups and with telemedicine preferred
  • Bilingual in English and Spanish preferred
