Social Worker M.S.W. at Shore Medical Center – Somers Point, New Jersey

Position Summary
The Social Worker is responsible for assessing, counseling, intervening, implementing and educating the patient, family/significant others to facilitate movement throughout the continuum of care.

Core Duties and Responsibilities
• Collaborates with patient, family and other team members to coordinate services
• Serves as a resource for community services
• Provides assistance in obtaining alternative living situations when needed
• Works with health care team, patient/family and community agencies in order to facilitate efficient utilization of resources
• Develops and maintains good working relationships with health and social service agencies
• Provides timely documentation of discharge planning activities in the medical record
• Educates patients, family/significant others regarding options for discharge to include what is available and what has been arranged.
• Provides discharge planning specifically for psychiatric/suicidal patients.
• Demonstrates an understanding of resource for Drug/Alcohol services and ability to refer appropriately.

Minimum Requirements
• MSW/ NJ Licensed Social Worker
• Previous clinical/Hospital experience
• Knowledge of Medical Terminology and intermediate computer proficiency
• Excellent organizational, communication and interpersonal skills
• Must be able to walk and stand for extended periods with the ability to lift, push and pull

Preferred Skills and Experience
• 2 years of experience in acute care

Salary Range - $54,000-$85,200 (based on experience)
