The Social Worker BSW provides essential psychosocial support and care coordination for patients and families. You would ensure timely follow-up, effective communication, and smooth transitions of care.

This role reports to the Director.

Essential Job Duties:

  • Conducts home and facility visits to assess patient and family needs in non-hospital settings
  • Performs comprehensive psychosocial assessments and develops appropriate care plans
    Collaborates with Clinical Care Coordinators, physicians, and care teams to manage complex cases
  • Coordinates care transitions, including follow-up visits, referrals, and transportation as needed
  • Screens for depression and ensures appropriate follow-up and interventions
    Communicates care plans effectively with patients, families, and interdisciplinary teams
  • Maintains accurate, timely documentation in medical and departmental systems
    Applies knowledge of healthcare benefits, community resources, and regulatory requirements
  • Demonstrates leadership in managing high-risk or catastrophic cases
    Participates in ongoing education, in-services, and community initiatives

Qualifications:

  • Strong knowledge of psychosocial assessment, care planning, and case management practices
  • Ability to manage complex cases and prioritize workload effectively
    Excellent communication and collaboration skills across interdisciplinary teams
  • Knowledge of healthcare systems, community resources, and continuum of care
  • Ability to interpret and apply federal, state, and organizational regulations
  • Strong documentation and organizational skills
  • Ability to work independently and make timely, informed decisions

Licenses / Certifications:

  • Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) or eligibility required
  • Valid driver’s license required
  • Basic Life Support (BLS) certification if required by organization
