Job Description

SUMMARY This position is responsible for HIV and STI/STD testing and education for those in incrassation locations two days a week and other out reach locations including persons at risk of acquiring or transmitting HIV and/or STI/STD. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: This position is responsible for providing prevention education to those at high risk for HIV or STI's, as well as case management of those already diagnosed with those infections. This will include a detailed assessment of individuals' risk factors for exposure, devising tailored plans for risk reduction, including the use of Pre-Exposure Prophylaxis (PrEP), and ongoing monitoring of the efficacy of high-risk behavior modification recommendations. This position is also responsible for providing linkage services to individuals newly diagnosed with STI's and or HIV as well as those with chronic infection in need of care. Key to these goals will be the promoting of the federal 340B program and enrolling clients into that program. The role also includes supporting patients with funding assistance applications, enrolling patients into the Affordable Care Act (ACA), Ryan White program (RW), or any other insurance in which the patient can qualify, direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. The role decreases barriers to medical and support services, increases consumers' awareness of treatment options, builds/strengthens relationships between the patient, physician, and patient advocate, and OTHER RESPONSIBILITIES Performs other work-related duties and special projects as assigned. Establish and maintain effective working relationships and with business community Conduct individual and group education presentation on topics related to HIV & STDs, safer sex practices, and HIV prevention education Organize events EDUCATION and/or EXPERIENCE Bachelor's degree from an accredited college or university or an associate degree and one (1) year of HIV or health education experience or a high school diploma or equivalent and two (2) years of HIV or health education experience. CERTIFICATES, LICENSES, and/or REGISTRATIONS None required.
