We are currently seeking a skilled and compassionate Licensed Practical Nurse to join our team in the Harrisburg/Lancaster/York, PA area.
Job Description:
As Hospice LPN you will be responsible for helping to manage and coordinate the care of patients in their homes, skilled nursing facilities, or assisted living environments. You will work closely with the RN Case Managers to assess patient needs and provide direct nursing care to ensure the comfort and dignity of patients at the end of life. This position requires a nurse with clinical expertise, compassion, and a commitment to providing high-quality hospice care.

You will work closely with the RN Case Managers to assess patient needs and provide direct nursing care to ensure the comfort and dignity of patients at the end of life. This position requires a nurse with clinical expertise, compassion, and a commitment to providing high-quality hospice care.
Key Responsibilities:
- Provide skilled nursing care including medication management, pain control, symptom management, and emotional support.
- Act as a liaison between patients, families, and the RN Case Managers to ensure coordinated care.
- Educate patients and families on disease processes, medications, and end-of-life care options.
- Monitor patient progress and adjust care plans as needed to ensure comfort and dignity.
- Document patient care and progress in compliance with hospice regulations and company policies.
- Participate in regular team meetings to discuss patient status and plan of care.
- Current and unrestricted LPN license in Pennsylvania
- At least 1 year of hospice or palliative care experience preferred
