Your Tailor Made Senior Service Home Health is urgently hiring a Physical Therapist (PT) or Occupational Therapist (OT) for a home health Veteran client located in Kaufman, Texas. This position is ideal for local therapists seeking PRN home health work close to home.

• Schedule: PRN / Per Visit

• Start Date: Immediate

• Local home health assignment – minimal drive time

• Serve and support a Veteran in their home

• Flexible scheduling that fits your availability

• Clinician-led, veteran-owned home health agency

• Opportunity for ongoing home health referrals

• Perform in-home PT or OT evaluations and treatments

• Develop and implement individualized plans of care

• Address functional mobility, strength, balance, ADLs, and safety

• Educate the patient and caregivers on treatment goals and home programs

• Complete timely and accurate home health documentation

• Communicate with the interdisciplinary care team as needed

• Current Texas Physical Therapist (PT) or Occupational Therapist (OT) license

• Home health experience preferred

• Experience with Veterans or geriatric patients is a plus

• Reliable transportation and valid driver’s license

• Ability to work independently in the home setting

• Strong clinical judgment and documentation skills
