Job Description

Job Purpose

We are seeking a highly organized and detail-oriented Scheduling Coordinator to join our team in the Program of All-Inclusive Care for the Elderly (PACE). The Scheduling Coordinator will play a crucial role in managing and coordinating appointments for participants of the PACE program, ensuring they receive comprehensive and timely care services. This position requires strong communication skills, multitasking ability, and a commitment to providing excellent customer service to elderly individuals.

Duties and Responsibilities

· Schedule and coordinate medical, therapeutic, and other health-related appointments for PACE participants in collaboration with healthcare providers and the interdisciplinary care team.

· Maintain accurate and up-to-date participant schedules, ensuring all appointments are scheduled in accordance with participants' needs and care plans.

· Communicate effectively with participants, families, and healthcare providers regarding appointment availability, changes, and cancellations.

· Monitor and track appointments to ensure all services are provided timely and efficiently, following up as necessary.

· Assist in managing transportation arrangements for participants, ensuring they have access to transportation for their scheduled appointments.

· Handle the rescheduling of appointments when needed, ensuring minimal disruption to participant care.

· Maintain detailed records of appointments, cancellations, and rescheduling within the program’s scheduling system.

· Collaborate with interdisciplinary teams to ensure that care plans align with scheduled appointments and services.

· Ensure that all appointment scheduling complies with program policies and regulatory requirements.

· Assist with reporting and data entry as required, providing updates on appointment status and trends.

· Act as a liaison between participants, families, providers, and internal teams to resolve any scheduling-related concerns or issues.

  • Other duties as assigned.

Qualifications

Education:

· High school diploma or equivalent required; Associate’s degree or higher preferred.

· Previous experience in healthcare scheduling, administrative support, or customer service, particularly in the elderly or senior care field, is preferred.

· Strong organizational skills with the ability to manage multiple tasks and prioritize effectively.

· Excellent communication skills, both written and verbal, with the ability to interact professionally with participants, families, and healthcare providers.

· Detail-oriented with the ability to maintain accurate records and handle confidential information.

· Proficiency with scheduling software and office tools such as Microsoft Office (Word, Excel, Outlook).

· Knowledge of the PACE program and its services is a plus.

· Ability to work independently as well as in a team-oriented environment.

· Compassionate and empathetic toward elderly individuals and their families.

  • Requires valid driver’s license.
  • Requires proof of automobile insurance coverage at the following minimum amounts in order to be reimbursed for mileage: $100,000/$300,000 personal liability and $100,000 property damage.
  • May require use of personal cell phone for business purposes (may be eligible for stipend)

Physical Demands

  • Must be able to remain in a stationary position 50% of the time.
  • Ability to occasionally move about inside the office to access file cabinets, office machinery, etc.
  • Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.
  • Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc.
  • The ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information is crucial for this role.
