Job Description

At Merrimack Health, we are committed to pay transparency and equity. The base salary range for this position is $22.66 – $33.99, excluding fringe benefits and potential bonuses.

Your final base salary will be determined based on your education, experience, licensure, and internal equity considerations. Offers are typically made below the top of the range to support future salary growth.

Eligible employees may also participate in comprehensive benefits, including health coverage, paid time off, and wage increase programs, depending on position type (full-time, part-time, or per diem) and applicable eligibility criteria.

Position Summary: Patient Navigator, 40 Hours per Week, Day/Evening Lawrence campus

The Patient Navigator works closely with the LIHPN leadership team, provider practices, ambulatory care sites, and patients to ensure all who need it have access to potentially life-saving cancer screenings, with particular focus on improving access for traditionally marginalized and underserved groups of people. Additionally, this role provides patients with low-barrier, culturally and linguistically appropriate education on the importance of obtaining timely cancer screenings in order to maintain overall health and wellness.

EDUCATION AND RELATED EXPERIENCE:

Required:
• High School Diploma/GED
• Experience working with Medicaid and other vulnerable populations and knowledge of the communities served by LGH
• Able to communicate effectively verbally and in writing
• Strong customer service skills
• Ability to work with adults of varying ages, learning styles, and backgrounds
• Proactive approach to addressing patient issues
• Ability to collaborate with multidisciplinary care teams and supportive services
• Capacity to develop strategies to resolve complex patient issues
• Knowledge of local health care systems and community resources
• Ability to work independently as well as in groups
• Computer proficiency, especially in Microsoft Word, Excel, Outlook and electronic medical records

Preferred:
• Bilingual, bicultural (Spanish/English)
• Associate’s or Bachelor’s degree in Human Services or a related field
• Two years’ experience in similar or related field
• Community Health Worker Certificate
