JOB TITLE: Per Diem Worker Service Support Professional (SSP)
REPORTS TO: Family Support Specialist
DIRECTLY SUPERVISES: N/A
JOB STATUS: Non-Exempt P/T- (5-40 hrs. as needed)-hired by family or care provider
LOCATION: As assigned. Telecommuter Exempt.
GENERAL SUMMARY: (General Purpose of the Job)
Habilitation/Companion/Transportation; Performs duties as delegated by Family Support Coordinator(s) in the community for persons with intellectual disabilities and/or behavioral health. Work with individuals on their individualized service plan (ISP) outcomes in their home or in the community.
ESSENTIAL FUNCTIONS: (Primary Responsibilities)
- To participate in the ongoing communication regarding program planning and implementation for individuals.
- Provide a safe, clean, comfortable and therapeutic environment during activities in the community.
- Encourage, guide and train individuals in developing daily living skills and independence skills in accordance with their ISP.
- Assist in organizing and participating in leisure/recreation activities.
QUALIFICATION STANDARDS:
- Education requirements as governed by regulations
- 18 years or older
- Valid Vehicle Insurance and Registration (100/300 Liability is required) If being reimbursed for transporting Individuals.
- Computer knowledge; Basic computer skills
- Physical ability to perform all duties associated with this position
- Properly preform lifting and carrying techniques. (Lift 25lbs)
- Valid U.S. Driver's License/ID
JOB REQUIREMENTS:
- Candidate will have ability to function in a team environment and ability to function independently. Must be organized and be able to follow directions and follow through with assigned tasks in a timely fashion.
- Candidate will have ability to work well with individuals and have the physical ability to assist any physical needs in the community (i.e. assist w/ wheelchair, assist w/ walkers, assist with incontinence accidents etc.).
- Candidate will have the ability to drive for extended periods of time and be in the community for extended periods of time.
- Candidate will have the ability to independently research community activities appropriate for the individuals they are working with.
- May be required, at times, to use personal vehicle for transport to community activities
- Telecommuter exempt
- Have a device to access and use EVV

Acting out behavior of individual could result in aggression.
Car Accident
