Job Description

Tasks/Duties/Responsibilities

70%

  • Conduct weekly social media postings about women and HIV
  • Create Educational Social Media sessions, and posts for PrEP, PEP, STI and HIV awareness.
  • Conduct social media virtual panel discussion to raise awareness of prevention activities and methods
  • Compile month reports for all self –testing activities under CDC (Centers for Disease Control) PS21-2102
  • Perform site visit to all subgrantee distribution sites
  • Conduct outreach 2-3 times a week and recruit individuals to engage in HIV/STI testing
  • Conduct pre- and post-test counseling services with customers to engage in dialogue surrounding HIV transmission, risk reduction options, and testing options.
  • Conduct HIV and STI testing, utilizing universal precautions and conforming to all state regulations regarding testing protocols.
  • Refer customers to appropriate follow-up services including, but not limited to: PrEP, nPEP, linkage to medical care, linkage to community health worker.
  • Prepare and submit required documentation and specimen for STI/HIV diagnosis
  • Complete testing kit logs, testing room temperature logs, and control logs
  • Monitor testing kits and specimen tube inventory
  • Refer customers to partner services, as appropriate.
  • Work with Community Health Worker to monitor referrals to STI treatment to ensure successful linkage to treatment.

Reporting /Monitoring

  • Compile monthly reports for all self-testing activities.
  • Oversee that customer case records using approved agency forms and procedures are maintained and updated. Ensure confidentiality of all records.
  • Track social media on-line activities such as HIV/STI appointment requests, like and shares of STI/HIV information, shares, new page members etc.
  • Prepare monthly and quarterly reports in accordance with agency policies and procedures.
  • Track and spend down quarter program budgets
  • Maintain and update client information in SharePoint, eCOMPAS and/or EvalWeb
  • Develop data update and present to Hyacinth leadership team and other stakeholders.

Hyacinth representation/advocacy

  • Attend and participate in staff meetings and supervisory conferences
  • Maintain working relationships with other community providers
  • Observe established administrative procedures.
  • Provide written reports and maintain statistics as required
  • Represent the agency at community events, meetings, and conferences
  • Attend and participate in staff training opportunities.
  • Adhere to Hyacinth policies and procedures including client confidentiality provisions

Training and certifications

· Attend NJDOH and required intervention trainings

· Attend NJ HIV Planning Group Committee meetings

· Participate on NJHPG committee

· Attend regional planning meetings and participate on one committee

· Attend any additional required community or DOH meetings

Total 100%

Required Minimum Education: bachelor's degree

Required Years of Related Experience: 2-4 years

A minimum of two years' experience in the field of HIV / AIDS
