About This Position

  • Position Summary
    • Reviews the patient’s medical records to assess appropriateness and medical necessity for admission and continued stay. Assists in identifying underutilization and over utilization of health care resources. Facilitates improvement in the overall quality, completeness, and accuracy of medical record documentation. Obtains appropriate clinical documentation and ensures that the clinical information is complete and accurate, in order to optimize reimbursement received for the level of services rendered. Educates members of the patient care team on documentation guidelines. Assists with scheduling and formulating initial treatment, discharge, after charge plans, and overseeing the medical needs of the patient in coordination with the attending physician and in collaboration with the clinical supervisor. Provides input into the development of the Master Treatment Plan. Works collaboratively with the Patient Care Team through regular communication and information sharing.
  • Minimum Qualifications
    • Work Experience
      • Three years recent clinical registered nurse, physical therapist, occupational therapist or speech therapist experience, in a home care setting required.
      • Case Management and Utilization Review preferred.

    • Education
      • Graduate of a registered nurse, physical therapy, occupational therapy or speech therapy school required.

    • Certifications/Licensure
      • Kentucky RN license (OR) RN license in a state recognized by the Kentucky Nurse Licensure Compact Act required effective June1, 2007, (OR) Licensed Physical Therapist in the Commonwealth of Kentucky, (OR) Licensed Occupational Therapist in the Commonwealth of Kentucky, (OR) Licensed Speech Therapist in the Commonwealth of Kentucky.
      • Valid driver’s license required.
      • Motor vehicle record check required and will be performed annually.
      • Personal vehicle is routinely used for company business, home visits or required for the position. The employee is responsible to maintain KY minimum limit of insurance coverage on the personal vehicle in good operating condition.
  • Job Specific Performance Standards
    • The duties listed below are a summary of the major essential functions of this position. The position may require other duties, both major and minor, that are not mentioned, and specific functions may change from time to time.
      • Works with clinicians to determine appropriateness of admission, continued stay, and readiness for discharge based on medical necessity following established guidelines of provider and home health policies.
      • Assists in identifying underutilization and over utilization of health care resources and is responsible for scheduling, organizing, and managing all aspects of a patient’s healthcare maintenance and treatment.
      • Confirms/Generates 485 once it is complete. Readies OASIS for encoding once reviewed and any corrections have been made.
      • Assesses SOC/ROC/Recert documentation along with therapy evaluations to assure OASIS documentation collaboration has been performed between clinicians per OASIS guidelines.
      • Performs follow-up on patients placed on hold whether at a facility or placed on hold per agency processes. Requests orders from physician for ROC when indicated. Assists in scheduling ROCs and evaluations post hospitalization.
      • Leads patient care conferences weekly and assures that clinicians are documenting communications per our Conditions of Participation (COPs).
      • Demonstrates knowledge in data collection, data analysis methodology, and performance methods needed to support and lead our Quality Assurance and Performance Improvement (QAPI) Program, and educates clinicians on identified opportunities for improvement.
      • Facilitates patient discharge from the agency and ensures that the discharge summary is sent to the physician within the timeframe set by the (COPs).
      • Covers for clinical supervisor and rehab supervisor as needed within licensing regulations.
      • Seeks out and attends classes/webinars to stay up to date on OASIS, current payment models, and any other home care specific items.
      • Participates in leadership and interdisciplinary team meetings while building collaboration with all parties at the table.
