Care Coordinator (Home Care) at Med Center Health – Bowling Green, Kentucky

Reviews the patient’s medical records to assess appropriateness and medical necessity for admission and continued stay. Assists in identifying underutilization and over utilization of health care resources. Facilitates improvement in the overall quality, completeness, and accuracy of medical record documentation. Obtains appropriate clinical documentation and ensures that the clinical information is complete and accurate, in order to optimize reimbursement received for the level of services rendered. Educates members of the patient care team on documentation guidelines. Assists with scheduling and formulating initial treatment, discharge, after charge plans, and overseeing the medical needs of the patient in coordination with the attending physician and in collaboration with the clinical supervisor. Provides input into the development of the Master Treatment Plan. Works collaboratively with the Patient Care Team through regular communication and information sharing.
