JOB REQUIREMENTS:
MINIMUM EDUCATION:
Practical Nurse licensed in the State of Illinois.
Current certification in Basic Life Support: Health Care Providers, Level C.
Evidence of successful completion of an approved pharmacology course.
MINIMUM WORK EXPERIENCE:
One year clinical experience.
Good interpersonal and communication skills.
Ability to function independently within scope of clinical practice.
REQUIRED LICENSES, CERTIFICATES, REGISTRATIONS: Practical Nurse licensed in the State of Illinois.

GENERAL SUMMARY/BASIC PURPOSE OF JOB:
The L.P.N. is responsible for the direct delivery of care to patients assigned to him/her under the supervision of the charge nurse. The L.P.N. works toward the improvement of care and quality of life for patients.
REPORTING RELATIONSHIPS:
 Reports to: Administratively to Nurse Executive, Unit Manager and Supervisor.
Clinically directed by Registered Nurse.
 Provides Supervision to: Certified Nursing Assistants
Staff Nurses in Orientation
Requirements/Providing Services to Groups for Specific Ages:
Completion of Annual Healthstream competency on Age Specific care
