SALARY: $60,000 plus additional $3,000 for independently licensed candidates, exempt
SCHEDULE: Monday-Friday, 8am-5pm, full-time
LOCATION: 6580 W. Indian School Rd, Phoenix, AZ, 85033
Work-from-Home Fridays!
You will get to the opportunity to work from home Fridays. This role requires telehealth and in-person appointments in the office or field. CLDF is committed to providing field-based services, meeting clients where they may be most comfortable.
ABOUT CPLC CENTRO DE LA FAMILIA (CDLF)
CDLF provides outpatient behavioral health services for families and individuals of all ages. The program offers counseling and therapy services to those living with serious mental illness, general mental health issues, and substance abuse issues. These issues can be related to depression, anxiety, relationships, loss, substance abuse, suicidal thoughts, and trauma from sexual, emotional and/or physical abuse. Outpatient services offer flexible office hours and home-based services for children and their families, public transit accessibility, therapy in English and Spanish, and professional staff who utilize best practice models for treatment.
HOW WILL YOU MAKE AN IMPACT?
You'll provide direct and indirect, covered services and treatment to individual clients, coordinating with community support and stakeholders. As a clinician, you'll be independent, managing your appointment schedule, assigned caseload, including conducting assessments, implementing treatment plans, facilitating out-of-office services, performing crisis services when needed, and participating in ongoing learning experiences and treatment team consultation and coordination.
- Actively engage clients and their families in their homes, community settings, or other field locations, tailoring your approach to meet them where they are most comfortable.
- Conduct comprehensive assessments and develop individualized treatment plans that empower clients to achieve measurable progress in their recovery journey.
- Lead individual and family counseling sessions and facilitate group therapy, creating safe spaces for healing and growth both in-office and out in the community.
- Collaborate with prescribers, medical providers, and pharmacies to support clients with medication and case management needs, bridging gaps in care through hands-on coordination.
- Maintain consistent communication and follow-up with clients, including welfare checks and re-engagement efforts, demonstrating your commitment to their progress and well-being.

- You understand lifespan development and harm reduction models, and you can confidently recognize and respond to a broad range of DSM diagnoses.
- You’re skilled in providing routine, urgent, and crisis interventions, applying motivational enhancement techniques and family systems approaches to foster lasting progress.
- You communicate with clarity and empathy, building trust with clients, families, and community partners while maintaining safety and cohesiveness in all settings.
- You’re highly organized and adaptable, managing priorities independently while ensuring accurate documentation and timely follow-up.
- You demonstrate cultural humility and respect, thriving in diverse, multicultural environments and embracing continuous learning to deliver exceptional care.
MINIMUM QUALIFICATIONS
- Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or related field from an accredited university.
- Licensed or eligible for associate licensure in Arizona
- If license eligible, must pass the licensing exam within 9 months of hire.
- Prior internship, practicum, or supervised clinical experience.
- Bilingual language skills in English/Spanish preferred.
- Ability to obtain fingerprint clearance card, CPR and First Aid.
- Ability to travel into the community as needed to meet client needs.
- Arizona Statutes Title 32 requires one to be at least 21 years of age to be licensed.
