POSITION DETAILS

Job Title: Patient Care Coordinator

Practice: Adult Joint Reconstruction, Knee & Hip Arthroplasty - Raymond Kim, MD

Status: This is a full-time, year-round, benefits eligible position.

Classification: Non-Exempt - Hourly

Location: This is an in-office position primarily based at our Vail location with some travel required to our other clinic locations.

Schedule: Monday - Friday

Pay Range: The entry range for this position is $25.00-$30.00/hr and is dependent on experience.

PATIENT CARE COORDINATOR OVERVIEW:

The Patient Care Coordinator is responsible for coordinating appointment times for patients and physician templates. This position requires you to obtain all required information from the patient, efficiently run a physician clinic, and learn how to schedule and coordinate surgical dates. The ideal candidate will be an excellent communicator who interacts courteously and professionally with individuals at all levels. Additional responsibilities include preparing and managing correspondence as directed.

CORE RESPONSIBILITIES:

  • Assist with scheduling clinic appointments (new patients, rechecks, etc), and patient referrals.
  • Confirms appointments with patient, ensures all documentation including registration and imaging prescriptions are in the system prior to the appointment
  • Administrative tasks may include triaging and returning patient phone calls, preparing FMLA and disability forms and other correspondence under the direction of the supervising physician, and assisting with other tasks related to patient care.
  • Schedules surgery including pre-operative and post-operative appointments.
  • Helps to coordinate add-on surgeries with surgical facilities and updates physician schedule.
  • Initiates the pre-operative process by scheduling surgery, pre-operative appointments and lab work.
  • Reviews all surgery instructions with patients. Fully educates patients about their procedures and ensure they understand their responsibility in obtaining necessary services prior to surgery.
  • Prepares preoperative data sheets.
  • Generate patient and staff correspondences as directed (creating/replying to ModMed tasks, etc)
  • Chart note prep for Physician and PA clinics (pending level of EMR experience/medical knowledge)
  • Assist with new patient image reviews (sending patient emails to obtain patient records/images)
  • Assist patients with logistical scheduling questions, concerns, and other inquiries related to practice.
  • Maintain knowledge of physician preferences and availability.
  • Works closely with clinical team on a daily basis to provide top patient care.
  • Establishes and maintains effective relationships with patients, families, key decision-makers, facility and department staff, and collaborating physicians.
  • Demonstrate knowledge and understanding of patient privacy rights.
  • Maintains confidentiality of all medical, financial, or other sensitive materials and information in printed, electronic or verbal form which may jeopardize the privacy of patients.
  • Accesses and uses the minimum necessary patient identifiable information to perform job responsibilities and duties and only for authorized purposes.
  • Maintain personal commitment to the Steadman Clinic mission and values.
  • Follow policies as established in the Employee Handbook.
  • Attend regular staff meetings and other required meetings.

Requirements:

MINIMUM QUALIFICATIONS:

  • Minimum of 1 year of experience working in a healthcare setting and/or hospitality.
  • Demonstrated problem solving and workflow management skills required.
  • Must be energetic, quality driven and have proven ability to be productive member of a cohesive team of peers dedicated to providing optimum patient care.
  • Must maintain a professional attitude towards patients and an excellent and collegial working relationship with other Steadman Clinic and hospital personnel in involved in patient care.
  • Must demonstrate understanding of procedures, policies and documentation required to ensure compliance with hospital standards of care.
  • Excellent critical thinking skills, ability to work independently, and manage time effectively.
  • Excellent organization, communication (both written and verbal), and interpersonal skills required.
