We're looking for a skilled, compassionate Home Infusion Pharmacist to join our clinical team and play a central role in improving patient outcomes across the Valley.

Schedule:
- Full-Time
- Monday–Friday, rotating shifts (9:30am–6pm and 10:30am–7pm)
- On-call rotation with a required response time of within 30 minutes
- Holiday rotation as scheduled
Key Responsibilities
- Independently perform all pharmacy tasks including compounding, pharmaceutical calculations, data entry, and general workflow — with hands-on proficiency in every function overseen or verified
- Perform sterile and non-sterile compounding independently in compliance with USP 797 and USP 800 standards
- Review and verify medication orders for accuracy, drug interactions, contraindications, and dosing
- Monitor patient drug therapy, interpret lab data, and adjust care plans to optimize outcomes
- Collaborate with physicians, nurses, and interdisciplinary care teams to coordinate patient care
- Educate patients and caregivers on medication use, safe administration, and what to expect
- Supervise pharmacy technicians and compounding personnel
- Use effective strategies to achieve daily goals, including prioritization, delegation, deferral, and reassignment of duties
- Demonstrate proficiency with pharmacy software and data entry systems
- Ensure full compliance with HIPAA, OSHA, state Board of Pharmacy, and accrediting body standards
- Participate in QAPI initiatives and support a culture of continuous improvement
- Perform other duties as assigned
Qualifications
Required:
- Doctor of Pharmacy (PharmD) or B.S. in Pharmacy
- Active, unrestricted Arizona Pharmacist license
- 1–2+ years of pharmacist experience
- Ability to independently perform compounding, calculations, data entry, and pharmacy workflow tasks
- Proficiency with pharmacy software and data entry systems
- Strong organizational and time-management skills with the ability to prioritize effectively in a fast-paced environment
- Strong communication and patient-facing skills
- Ability to participate in on-call rotation with a 30-minute response time and holiday rotation as scheduled
Preferred:
- Home infusion or sterile compounding experience
- BCSCP certification
- Familiarity with USP 797 and USP 800 compliance
- Experience in a collaborative, interdisciplinary or home health setting
