SummaryThe Patient Records Specialist is a key member of our healthcare team, responsible for managing patient records, ensuring seamless communication between our clinic and other healthcare facilities, and providing high-quality customer service. This role involves requesting, receiving, and accurately entering patient medical records into the Electronic Medical Record (EMR) system. The Patient Records Specialist also handles patient phone calls and other duties as needed in accordance with the philosophy, goals and objectives of Cozad Community Health System.
Essential Duties and ResponsibilitiesThis description intends to describe the general nature and level of work performed by employees assigned to this job. · Requests and tracks the receipt of medical records from other healthcare facilities.
· Prepares charts for scanning and scans charts into the electric medical record system, forwarding documentation to providers as needed.
· Provided clerical support to HIM staff in relation to filing and retrieval of medical records. Assists in other department functions as assigned, such as photocopying, faxing, typing, and any other departmental task as required by the HIM Director.
· Reviews and enters pertinent patient information into the EMR system accurately and in a timely manner.
· Ensures that all medical records are complete, organized, and updated according to the clinic protocols and legal requirements.
· Serves as a point of contact for patients when applicable.
· Responds to requests for copies of protected health information, via fax, email, phone, and in-person in accordance with HIPAA privacy and security regulations, and CCHS policies and procedures.
· Maintains patient medical records, maintains storage, and tracks incoming records.
· Promotes and ensures patient safety in all aspects of responsibilities.
· Provides a purposeful patient and family centered partnership with the healthcare team that facilitates a seamless patient experience.
· Works with other departments to provide services and resources.
· Participates in training to stay current on Rural Health Clinic regulations.
· Provides quality healthcare services to all patients.
· Follows all HIPAA guidelines and promotes and ensures patient safety.
· Fulfills mission statement by performing job duties with a high level of customer service while contributing to a positive and professional environment.
· Special projects and other duties as assigned.
Requirements: Education and/or ExperienceElectronic Health Records experience preferred.
Licensure/CertificationBasic Life Support (CPR)

Ability to operate a motor vehicle and complete errands based on business needs.
Work EnvironmentWhile performing the duties of this job, the employee regularly works in a medical clinic setting. The noise level in the medical clinic environment is usually moderate.
