Key Responsibilities

  1. Assess clients’ individual needs and assist in planning, developing, and implementing appropriate support and care programs within a residential disability setting.
  2. Interview clients to identify the nature and extent of their personal, social, and behavioural difficulties.
  3. Monitor, evaluate, and report on clients’ progress to ensure that support plans remain responsive to their changing needs and aligned with their NDIS goals.
  4. Refer clients to external agencies and community organisations that can provide additional or specialised assistance.
  5. Assess community needs and available resources relating to health, welfare, housing, employment, training, and other essential services relevant to persons with disabilities.
  6. Liaise with families, community groups, welfare agencies, government bodies, and service providers to coordinate care and promote awareness of available resources and supports.
  7. Provide direct care and assistance to clients with disabilities in residential facilities and supported accommodation, ensuring safety, comfort, and participation in daily activities.
  8. Supervise clients within the residential setting to ensure safety, wellbeing, and compliance with individual care or behaviour support plans.
  9. Support families in managing care responsibilities and promote the wellbeing and social inclusion of residents.
  10. Assist clients in developing practical life skills and strategies to address social, emotional, and financial challenges.
  11. Prepare and maintain accurate case notes, progress reports, and funding submissions in accordance with organisational policies and regulatory requirements.
  12. Contribute to a positive and collaborative workplace culture by supporting colleagues and sharing best practices in service delivery.

Qualifications and Experience

The ideal candidate will have:

  1. A relevant qualification in community services, disability support, aged care, or a related field
  2. Experience working in residential care, supported accommodation, or community-based disability services
  3. Sound understanding of person-centred care principles and the NDIS framework
  4. Strong organisational and time management skills, with the ability to manage multiple client needs and documentation requirements
  5. Proficiency in completing case notes, progress reports, and care-related documentation accurately and efficiently
  6. Excellent communication and interpersonal skills, with the ability to build positive relationships with clients, families, and colleagues
  7. Compassion, patience, and a commitment to promoting the independence and dignity of people with disabilities
  8. Knowledge of health and safety, infection control, and safeguarding standards within a residential care setting
  9. A proactive and reliable attitude, with the ability to work both independently and collaboratively within a multidisciplinary team
