  • Coordinate services across behavioral health, medical, housing, employment, education, and community resources.
  • Advocate for clients with healthcare providers, schools, courts, social service agencies, and other community organizations.
  • Connect individuals to benefits, community resources, and supportive services that promote independence and wellness.
  • Help clients navigate crises by providing support, intervention, and linkage to appropriate services.
  • Monitor client progress and adjust care plans as needs change.
  • Maintain timely, accurate documentation that meets Medicaid, regulatory, and organizational standards.
  • Participate in treatment team meetings, supervision, and ongoing professional development.
  • Travel throughout the community to meet clients where they are, including providing transportation when appropriate.
  • Promote culturally responsive, trauma-informed, and person-centered care in every interaction.

  • Build strong partnerships with community organizations and referral sources.
  • Meet productivity and documentation expectations while delivering high-quality services.
  • Stay current on best practices, regulations, and professional standards.
  • Foster collaboration, teamwork, and a positive workplace culture.
Requirements: What We're Looking For
Education
  • Bachelor's degree in Social Work, Psychology, Rehabilitation, Human Services, or a related behavioral health field.
  • Equivalent education and relevant experience may be considered in place of a degree.
Experience

Preferred qualifications include:

  • Experience working in behavioral health, community mental health, outreach, or case management.

  • Excellent organizational and time management skills.
  • Experience coordinating care across multiple systems and community providers.
  • Confidence facilitating meetings and advocating on behalf of clients.
  • Strong documentation and computer skills, including Microsoft Office.
  • Cultural awareness and the ability to work respectfully with diverse populations.
  • A commitment to ethical, person-centered, and strengths-based care.
Requirements
  • Valid, non-expired driver's license.
  • Reliable transportation with current comprehensive automobile insurance and a satisfactory driving record.
  • Willingness to travel throughout the service area and transport clients as needed.
  • Ability to successfully complete required background screenings, including criminal history and Kansas Child Abuse/Neglect Registry clearance.
  • Ability to perform the essential functions of the position with or without reasonable accommodation.
