Full Time Primary Care Advanced Practice Provider Position Available - Outpatient
Columbus Regional Health, an independent health system with a membership to the Cleveland Clinic Connected program, is seeking a Full Time (1.0 FTE) Board-Eligible or Board-Certified Advanced Practice Provider to join the Doctors Park Family Medicine team in Columbus, Indiana.

To be hired as a Nurse Practitioner:
- Master's Degree in Nursing
- Current RN license in the state of Indiana
- Active unlimited license from the IPLA State Board of Nursing for Advanced Practical Nursing
To be hired as a Physician Assistant:
- Graduate of an approved Physician Assistant Training program
- Unlimited Physician Assistant license from the Indiana Medical Licensing Board
