Job Description
Position Summary
The Special Child Health Services Unit Case Manager provides ongoing case management to children ages 0-22 who have special healthcare needs. Areas assessed for each family include: medical, developmental, rehabilitative, education and social, emotional, and economic domains.
Primary Position Responsibilities
1. Creates individual service plans for each child identified as requiring Case Management Services.
2. Contacts new referrals from sources of new referrals: Birth Defects Registry and Autism Registry. Other referral sources include the Social Security Administration, The CICRF, health care providers and local agencies.
3. Provides ongoing support and to families; reviews individual service plans for each client.
4. Advocates for clients with local, state, federal and other agencies/resources.
Qualifications
Minimum Education:
Required:
Bachelor’s Degree in Nursing or Master’s Degree in Social Work

Minimum Years of Experience (Amount, Type and Variation):
Required:
● Two (2) years experience in public health and/or pediatrics (hospital, clinic,EIP, etc.) including supervisory experience.
● A MSN may be submitted to 1 year of experience OR a social worker with a Master’s Degree in social work and one (1) year experience in public health, or with handicapped or chronically ill children, including supervisory experience.

● New Jersey RN licensure or LCSW.
● Valid Driver’s License.

Required:
Knowledge of local, state and federal resources, computer and phone skills as well as exceptional customer service skills
