Summary of position

The Team Leader, Community Navigator oversees the daily operations of Community Navigator staff, ensuring effective delivery of case management, employment, and training services. Reporting to the Executive State Director, this role plans, assigns, and evaluates staff work, provides ongoing coaching and training, and contributes to hiring and staffing decisions.

The Team Leader manages a team of community Navigators and provides clinical guidance and support on complex cases. This position supports program improvement by reviewing and recommending updates to policies, procedures, and service delivery practices. This position maintains accurate records to support operational effectiveness.

The role collaborates with provider agencies to ensure timely and efficient service delivery and represents the program in meetings, committees, and community engagements. The Team Leader also communicates with clients, families, partner agencies, and the public—explaining program policies, addressing concerns, and fostering positive community relationships.

Essential Functions

  • Leadership & Staff Management
    • Recruits, supervises, and mentors Community Navigators; provides ongoing coaching, technical guidance, and corrective action as needed.
    • Establishes goals and performance expectations; conducts monthly supervision and annual evaluations, leveraging performance tools such as the Playbook scorecard.
    • Plans, assigns, and monitors staff workflow to ensure effective service delivery, productivity, and compliance with regulatory requirements.
    • Provides caseload coverage during staff transitions or absences.
    • Fosters a collaborative, respectful, and inclusive team environment that supports high‑quality customer service.
  • Program Oversight & Compliance
    • Ensures staff adherence to company policies and all local, state, and federal regulations governing waiver‑based services.
    • Monitors performance and contractual deliverables, ensuring accuracy and timeliness of mandated reports.
    • Implements corrective action plans when service delivery or compliance issues are identified.
    • Ensures readiness for accreditation and quality assurance reviews, including CARF and other required standards.
  • Program Planning, Evaluation & Reporting
    • Participates in program planning, implementation, and evaluation to ensure alignment with organizational goals and applicable rules, regulations, and policies.
    • Collects, analyzes, and reports program data to assess performance and support decision‑making.
    • Provides regular reports to leadership and contributes to the development of program goals, measurable outcomes, and annual work plans.
    • Collaborates with the State Director and Quality Enhancement Coordinator to review program evaluations and implement improvements.
  • Financial & Administrative Responsibilities
    • Provides input on annual operating budgets and monitors resource utilization.
    • Reviews and verifies staff reimbursement and travel requests and routes supply needs to the Director.
    • Completes required monthly supervision documentation for upload to UltiPro.
  • Partnerships, Outreach & Business Development
    • Strengthens relationships with providers, community partners, and state agencies to support coordinated service delivery.
    • Represents the organization in meetings, committees, conferences, and outreach events to promote programs and expand visibility.
    • Contributes to identifying new funding opportunities, supporting contract growth, and leveraging provider and customer relationships.
    • Assists in developing strategies for program expansion and continuous improvement.
    • Conducts community outreach activities per the Team Lead Outreach Plan.
  • Other Duties
    • Performs all other duties as assigned by the State Director or authorized leadership.

Position Requirements

  • Associates Degree in Health, Human Services, or a related field; OR a certificate in Medical Assisting, EMS, Counseling, Therapeutic Services, Healthcare Administration, or a related field; OR at least one (1) year of experience in health or human services involving interviewing and assessing individual needs.
  • Prefer Bachelor's degree in Health, Human Services or Social Work.
  • Three (3) years of case management experience, including assessing, planning, implementing, monitoring, and evaluating service options.
  • Three (3) years of experience making service plan recommendations, including treatment, counseling, or eligibility determinations.
  • Experience interpreting and applying relevant laws, regulations, policies, and procedures.
  • Experience using automated information systems for data entry, documentation, and reporting.
  • Experience preparing clear and accurate narrative reports.
  • Valid state driver’s license, current automobile insurance, and reliable transportation required.

Skills, Knowledge & Abilities

  • Proficient in Microsoft Office 365 applications.
  • Knowledge of planning principles, report development, and data analysis.
  • Skilled in interpreting and applying policies, procedures, and regulatory requirements.
  • Strong analytical and problem-solving skills, with the ability to evaluate situations, assess information, and recommend appropriate actions.
  • Demonstrated ability to build and maintain effective relationships with staff, partner agencies, and stakeholders.
  • Competence in clarifying rules and regulations and providing clear guidance.
  • Excellent written communication skills, including reports and professional correspondence.
  • Effective verbal communication, including presentations and responding to diverse audiences.
  • Skilled in defining problems, collecting and analyzing data, and drawing sound, evidence-based conclusions.

Travel/Work Environment

  • Mobile role requiring regular travel to multiple work locations within the assigned territory.
  • Work may involve exposure to typical outdoor weather conditions and extended periods of standing or sitting.
  • Residency within the assigned territory is expected; exceptions may be made based on business needs.
  • Occasional evening and weekend work may be required to meet operational or program demands.
