The Rehabilitation Supervisor is responsible for providing clinical leadership, supervision, and operational oversight of the rehabilitation department, including Physical Therapy, Occupational Therapy, Speech Therapy, and Physical Therapist Assistants. This position ensures the delivery of high-quality, patient-centered rehabilitation services in accordance with Medicare Conditions of Participation, California regulations, and agency policies.
The Rehabilitation Supervisor provides direct patient care while supervising rehabilitation staff, supporting clinical excellence, monitoring productivity, ensuring documentation compliance, and promoting positive patient outcomes.
Reports To: Director of Patient Care Services (DPCS) / Clinical Director
Supervises:
- Physical Therapists (PT)
- Physical Therapist Assistants (PTA)
- Occupational Therapists (OT)
- Speech-Language Pathologists (SLP)
- Supervise and mentor all rehabilitation clinicians.
- Serve as the clinical resource for rehabilitation staff.
- Promote evidence-based rehabilitation practices.
- Ensure quality patient care and achievement of functional outcomes.
- Participate in interdisciplinary case conferences and care coordination meetings.
- Support the agency's Quality Assurance and Performance Improvement (QAPI) initiatives.
- Conduct orientation and onboarding for rehabilitation staff.
- Provide ongoing coaching, mentoring, and performance evaluations.
- Perform annual competency assessments.
- Conduct supervisory field visits and chart reviews.
- Assist with recruitment, interviewing, hiring, and retention of rehabilitation staff.
- Monitor clinician productivity and caseload distribution.
- Review therapy evaluations, reassessments, recertifications, and discharge documentation.
- Ensure compliance with Medicare Conditions of Participation and agency policies.
- Monitor OASIS accuracy as it relates to therapy services.
- Assist clinicians with complex patient cases.
- Ensure timely completion of documentation.
- Review physician orders for appropriateness and compliance.
- Monitor rehabilitation documentation for quality and regulatory compliance.
- Participate in chart audits and utilization reviews.
- Monitor patient outcomes and functional improvement measures.
- Identify opportunities for clinical improvement and education.
- Assist with survey readiness and regulatory compliance activities.
- Participate in corrective action planning when needed.
- Collaborate with Intake, Scheduling, Clinical Managers, and Case Managers to ensure timely patient admissions and therapy visits.
- Monitor staffing levels and assist with coverage planning.
- Coordinate therapy services across multiple branch locations as assigned.
- Assist leadership with department budgeting and resource planning.
- Evaluate productivity metrics and operational efficiency.
- Maintain a reduced patient caseload as assigned.
- Complete evaluations, reassessments, and treatment visits.
- Develop individualized treatment plans.
- Coordinate care with physicians, nurses, social workers, and other disciplines.
- Educate patients and caregivers regarding treatment plans, safety, fall prevention, and home exercise programs.
- Assist in developing rehabilitation policies and procedures.
- Prepare departmental reports and productivity metrics.
- Participate in leadership meetings.
- Support accreditation and licensing surveys.
- Maintain departmental equipment and educational resources.
- Graduate of an accredited Physical Therapy program.
- Doctor of Physical Therapy (DPT) preferred.
- Current California Physical Therapist License in good standing.
- Current CPR/BLS Certification.
- Valid California Driver's License and reliable transportation.
- Minimum of three years of Home Health Physical Therapy experience.
- Minimum of one year in a leadership, supervisory, or clinical coordinator role preferred.
- Experience with Medicare-certified Home Health agencies.
- Strong knowledge of OASIS documentation and Medicare Conditions of Participation.
