The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

As a Patient Navigator, you'll help families navigate the process of obtaining an Initial Developmental Evaluation and connect them with appropriate therapies and services along the way. From completing patient intakes and coordinating screenings to following up with families and tracking outcomes, you'll help make the path to care clearer and more manageable.

As a Patient Navigator, a typical day might include the following:

  • Provide intake for patients seeking Initial Developmental Evaluations, including reviewing current services and diagnoses.
  • Help families understand and identify therapies or services that may be accessed while they await evaluation.
  • Assist with patient intakes, mailings, and scheduling for additional services as needed.
  • Register children for developmental screening activities.
  • Conduct follow-up phone calls with families after developmental screenings.
  • Enter and track screening-related data and monitor outcomes.
  • Work collaboratively with a variety of organizations and personnel to support children and families.
  • Perform other activities and responsibilities as directed by the Supervisor.

  • Schedule: Day shift, Monday through Friday 9:00am-5:00pm, Full-Time, 40 hours per week

To be considered for this role, you must have a minimum of one year of experience in early childhood systems of care or other disability-related activities. Candidates should have knowledge of early childhood developmental diagnoses, including speech delay, autism, and global developmental delay. While a bachelor's degree is preferred, strong communication, organizational, and time-management skills are needed, along with the ability to work effectively with a variety of organizations and personnel with compassion, tact, and clarity. Familiarity with Microsoft Excel and the ability to learn and use Electronic Medical Record systems are also important. Spanish-English bilingual skills are strongly preferred.
