Position SummaryThe Hospice CNA provides personal care services to hospice patients under the supervision of a Registered Nurse and in accordance with physician orders and individualized care plans. This role focuses on enhancing patient comfort, promoting dignity, and supporting families during end-of-life care.
Essential Responsibilities- Provide personal care assistance, including bathing, dressing, grooming, toileting, and mobility support.
- Observe and report changes in patient condition, physical status, and emotional well-being to the hospice care team.
- Document patient care activities and relevant observations accurately and timely.
- Follow established hospice care plans and nursing instructions.
- Assist patients in maintaining comfort and quality of life.
- Support family members and caregivers through compassionate communication and care.
- Maintain regular communication with the RN, Clinical Coordinator, and interdisciplinary hospice team.
- Participate in staff meetings, training programs, and continuing education opportunities.
- Complete scheduled patient visits in accordance with agency standards and patient needs.
- Current Virginia Certified Nursing Assistant (CNA) certification required.
- Valid driver's license, reliable transportation, and auto insurance.
- Ability to travel to patient homes throughout the service area.
- Strong communication and interpersonal skills.
- Compassionate approach to caring for patients and families facing end-of-life challenges.
- Hospice, home health, or long-term care experience preferred.
