Job Summary
The Targeted Case Management (TCM) lead case/care manager position provides ongoing leadership assistance to the Clinical Coordinator and provides additional support and guidance to the team in their absence. In addition, this position offers support to adults with co-occurring substance use and serious/persistent mental illness in the community. Various services provided include symptom monitoring, ongoing assessment and treatment planning, crisis intervention services, case management, and medication monitoring.


Essential Duties and Responsibilities
• Provide ongoing leadership assistance to the Clinical Coordinator and in their absence, act as the leader of the team – lead meetings as scheduled, provide guidance to team as needed
• Exhibit knowledge of evidence-based practices such as motivational interviewing and trauma informed care
• Collect all client staffing sheets for TCM psychiatric appointments
• Assist in writing Chapter 51 Commitment extensions
• Assist in reviewing continuation paperwork (ex. crisis case management paperwork, treatment plans, SARJs, crisis plans, etc.)
• Track team required forms and update appropriate spreadsheets
• Collect all clients My Home documentation every month
• Review and submit extra check request forms
• Assist in scanning and uploading documents for BHS review
• Collect risk assessment forms every month
• Maintain a smaller caseload (20-22) and participate in regular case manager duties:
o In an efficient/effective manner, case managers are responsible for locating, managing, coordinating, and monitoring all services and informal community support needed by participants and their families (may include assessment, case planning, monitoring and review, advocacy, and referral)
o Aid participants with activities of daily living to include coaching or finding resources in the areas of housing, independent living skills, money management, vocational/educational pursuits, scheduling and transportation to appointments, etc.
o Act as the single point of contact by coordinating the efforts of all professionals and paraprofessionals involved with each individual and mobilize resources in and outside the office to assist in achieving treatment plan goals
o Conduct home visits and transport participants in the community, using personal vehicle or agency van as needed
o Maintain accurate and timely documentation in the form of assessments, community treatment plans, progress notes, etc. as required
o Responsible for formulation of appropriate discharge planning
o Assist participants with money-related issues and coordination with payee services
o Encourage and develop natural supports for participants, including family members, neighbors, friends, etc.
o Attend and participate in staffings and in-services/team meetings, mandatory trainings, and consultations with the APNP and TCM Psychiatrist
o First shift work (8am-4pm), but position does require some extended hours
o Assist with participant coverage for co-workers as needed/directed
o Other job-related duties that may be necessary to carry out the responsibilities of the position

Required Qualifications
• Bachelor’s or Master’s Degree and 1-2 years social work experience preferred
• No convictions or license revocations that would fail a caregiver background check
• Valid driver’s license, automobile, and sufficient insurance to meet agency requirements


Knowledge, Skills and Abilities
• Good clinical skills, especially related to mental health and substance use disorders
• Knowledge of DSM V, psychotropic medications, and their side effects
• Ability to communicate and work effectively in a positive manner with a diverse population of staff and participants
• Possess excellent leadership skills to be able to cross train and/or cover for the Clinical Coordinator as needed
• Ability to respond appropriately in crisis situations, including de-escalation
• Demonstrate cultural sensitivity, professionalism, and strong interpersonal skills while representing the agency both internally and externally
• Ability to manage change, think critically, and be solution-oriented when dealing with situations as they arise in a fast-paced environment
• Good organizational and computer/typing skills
• Effective time management skills and the ability to multitask to meet various deadlines
• Commitment to the case manager role and the participants we serve
• Ability to set professional boundaries with ethics and integrity
• Work as a team member who participates productively while also managing independent duties and responsibilities
• Ability to assist participants minimally or set them up with resources that can assist with activities of daily living such as shopping, cleaning, and cooking
• Sense of humor

PHYSICAL DEMANDS:
Position is based in an office setting with about 60% of the time in the community, including transporting participants and home visits; must be able to go up and down stairs and lift/carry 20 pounds to periodically assist participants with various tasks. WORK ENVIRONMENT:
Work is performed both in a busy treatment environment, as well as in the community where participants’ symptomatology may be evident; some of the work is done sitting at desk using a computer; requires outreach in the community and clients’ homes; much of the outreach is done in the inner city/Milwaukee County; up to 5 hours per day may be spent driving; hours average 40 per week, but a flexible schedule is required to meet the needs of staff and those we serve.
