Job Description
The Team Mentor provides leadership, guidance, and hands-on support to direct support professionals (DSPs) in delivering high-quality, person-centered care to individuals with developmental and/or intellectual disabilities in community living settings. The Team Mentor serves as a role model, coach, and liaison between staff and management, ensuring that daily operations align with the organization's mission and values. The Team Mentor will oversee a caseload of 10-15 participants, while working directly with those and other participants.
We're looking for someone who naturally steps up to help others, leads by example, and wants to see both participants and coworkers succeed. The Team Mentor should be someone staff can go to for support, guidance, and problem-solving while still knowing when to involve a supervisor.

Position Reports to: Lead Opportunities Director. Holocracy model requires transparency and collaboration with other team members as well.
Primary Responsibilities:
- Mentorship and staff support: Provide ongoing coaching, training, and modeling of best practices for direct support staff.
- Promote a positive team culture through regular communication and problem-solving.
- Individual Support & Advocacy: Ensure individuals receive support that respects their rights, preferences, and goals.
- Assist with developing and implementing Individual Customized Plans.
- Monitor individual progress and well-being, and report concerns promptly.
- Understand, advocate, and support community inclusion and community employment for participants.
- Operational Duties: Support scheduling, shift coverage, and day-to-day operational needs
- Provide at least 32 hours of direct support while maintaining accurate and timely documentation related to service delivery
- Model ethical and professional behavior at all times.
- Effective communication within teams and stakeholders.
Primary place of work: Community. There is some travel between participant’s homes when doing orientation, training, technical assistance with participants and direct support professionals. Reimbursement of an additional $2.00 per direct hour is included for this. Travel outside of the Greater Des Moines area is not required nor expected. If an out of state training opportunity arises, it will be up to the discretion of the employee if that would work or not for their personal situation. All required trainings for the position currently are held in the greater Des Moines area.
Requirements:Minimum Requirements
- High school diploma or GED.
- At least 1 year of experience working with people with disabilities, in human services, caregiving, community services, or a related field.
- Demonstrated ability to work effectively as part of a team.
- Strong communication and interpersonal skills.
- Reliable attendance and punctuality.
- Ability to maintain professional boundaries and confidentiality.
- Ability to model Hope Agency's values and expectations for other team members.
- Ability to provide guidance, encouragement, and constructive feedback to coworkers.
- Ability to remain calm, flexible, and professional when handling challenging situations.
- Ability to prioritize participant safety, dignity, choice, and independence.
- Ability to complete required documentation accurately and on time.
- Ability to learn and follow Hope Agency policies, procedures, and participant-specific plans.
- Must be able to pass required background checks and meet all applicable employment requirements.
- Valid driver's license and reliable transportation, if driving participants is part of the position.
- Previous experience as a DSP, job coach, direct care worker, caregiver, or similar human-services role.
- Previous informal or formal leadership experience.
- Experience training, onboarding, or mentoring coworkers.
- Experience working with individuals with intellectual/developmental disabilities.
- Two or more years of experience in human services.
