PRISM Outreach Navigator at Hyacinth Foundation A New Jersey – Paterson, New Jersey

Position Summary

The PRISM Outreach Navigators for Passaic County and Hudson County, NJ implements culturally responsive, trauma-informed outreach, education, community assessment, and navigation services for LGBTQIA+ individuals. The position supports PRISM SMART Objectives and ensures compliance with NJDOH and Hyacinth AIDS Foundation requirements.

Key Responsibilities

· Conduct street-level, venue-based, and community outreach to LGBTQIA+ populations

· Administer multilingual community surveys; assist with focus groups and key informant interviews

· Plan and facilitate PRISM Talks, workshops, and Minority Health Month event

· Participate in county-level health fairs and community events

· Distribute culturally and linguistically appropriate educational materials

· Provide individualized navigation and linkage to HIV/STI testing, PrEP, mental health, housing, and harm reduction services

· Conduct follow-up, barrier assessments, and appointment reminder

· Document all services within 48 hours and submit required reports on time

· Attend all required NJDOH meetings, trainings, and site visits

Qualifications

· Commitment to harm reduction and LGBTQIA+ supportive services

· Bachelor’s degree with 2+ years of HIV prevention or outreach experience OR 4+ years relevant experience

· Strong communication and documentation skills

· Ability to work evenings/weekends as needed

· Bilingual English/Spanish preferred

· Valid driver’s license and reliable transportation preferred
