About This Position

Summary:

The Family/Community Liaison will work under the supervision of the Program Manager to conduct Family workshops to enhance communication skills for the purpose of substance misuse prevention and Family/child communication regarding risky behaviors. He/ She will conduct outreach to target communities to promote the importance of educating life span populations on substances, prevention, harm reduction, treatment, and recovery through education presentations, and outreach events, etc.

  • Work with Elmcor’s school partners; CBO and FBO partners to provide information sessions designed to educate the community at large for the purpose of substance misuse prevention.
  • Provide outreach services and conduct presentations in the community to inform others of RISE UP goals and programming.
  • Establish and maintain effective working relationships with CBOs and FBOs providing housing, substance use treatment, legal, health, mental health, educational, vocational, and other support services for youth and adults.
  • Provide outreach training workshops and team building activities for Elmcor peer mentors – Prevention, Treatment and Recovery programs. .
  • Refer and assist in scheduling appointments for program participants to family support services
  • Provide and/or assist with arranging translation, transportation, and other practical support services.
  • Assist in meeting the goals of the Prevention Coalition and Queens Regional Network.
  • Assist in developing the Family corner for Newsletter.
  • Attend workshops/conferences and represent Elmcor in the service provider community at various meetings and functions.
  • Assist the Program Manager in developing and improving program services to meet or exceed all contracted and/or mandated objectives and goals.
  • Complete and submit all required workshop evaluation summary reports, pre and post tests on a monthly basis.
  • Oversee and assist with all intervention and prevention services which may include: community service projects, cultural and educational enrichment trips etc. including monitoring attendance and the maintenance of participant records.
  • Facilitate individual and group work, prevention and intervention activities, family support services, educational sessions and social/recreational/community service activities.
  • Provide and follow up on referrals to community services providers, including advocating for youth and problem solving with community agencies and health providers.
  • Attend all meetings, trainings, collaborative efforts and other program and agency-related functions.
  • Travel to off site locations to provide services.

  • High School Diploma and 3-4 years experience,
  • Bachelors in social work, human service, community health, public health or related fields preferred.
  • Position requires knowledge of Family/Community education and advocacy issues and development; demonstrated experience and knowledge of family service delivery programs and the community at large; conducting workshops; knowledge of substance use prevention, program implementation and evaluation are required; and effective interpersonal skills required.
  • Position has a flexible schedule that includes some evening/weekend work. Must be computer literate, bilingual a plus.
