Behavioral Health Specialist at Holyoke Medical Center – Holyoke, Massachusetts

Maintain a caseload on the inpatient behavioral health unit which includes conducting patient assessment, identifying patient needs, collateral contact with families and outpatient providers, risk assessment and discharge planning. Discharge planning includes making necessary referrals, connecting the patient with necessary community resources, coordinating discharge plans with family or providers and obtaining follow-up appointments with current providers. Will also provide psycho-education, brief therapeutic interventions and daily support to patients. Works with a multidisciplinary team and communicates effectively with the team regarding the patient's progress, needs and discharge plans. Daily progress notes and treatment plan updates are also required.
