Nurse Navigator Cardiovascular at St. Clair Hospital – Pittsburgh, Pennsylvania

Summary

Works in partnership with physicians, hospital personnel, patients, significant others and pre/post-acute services to educate, coordinate, and facilitate the care of patients with cardiovascular conditions. Responsible for the organization and analysis of data supporting cardiovascular initiatives. Completes abstraction and data entry into required databases. Develops and generates outcomes report and acts as a resource for members of the Division of Cardiovascular Services. Facilitates orientation of new staff and planning for service line events and new procedures.

Minimum Qualifications

1. Current licensure as a Registered Nurse in Pennsylvania.
2. Bachelor’s degree in nursing.
3. Three years of experience planning and implementing care for patients undergoing structural heart procedures.
4. Knowledge and experience in data management in a healthcare setting with a focus on clinical analysis.
5. Background in advanced spreadsheet and database software programs, statistics, and clinical data abstraction.
6. Effective verbal and written communication skills.
7. Ability to address complex issues with patients, family members, and staff across multiple departments and community providers.
8. Ability to work independently and to gather pertinent patient information.

Preferred Qualifications

Master’s degree in nursing or health-related field.
