Rehab Case Coord at Parkview Health – Fort Wayne, Indiana

Must be a graduate of professional school of nursing and hold at a minimum an Associate of Science in Nursing (ASN). Must obtain Bachelor of Science in Nursing (BSN) within three years of employment or will not be eligible to continue in the role. Coursework is completed outside of work hours. Must provide current RN license by Indiana State Board of Nursing. CRRN certification preferred. Experience in acute rehabilitation program preferred. Must be able to lift up to 50 pounds.
