SUMMARY
Foundationally, the Program Coordinator assumes all of the same clinical responsibilities as does a Primary Therapist, and engages in the clinical work of the program for a percentage of time, determined by Behavioral Health administration. Essentially, the Program Coordinator provides operational and clinical leadership to program staff contributing directly and indirectly to patient care. Provide superior customer service.
STATUS: Full-Time, 40 hours per week, FTE: 1.0
LOCATION: Unity Hospital; 1555 Long Pond Road, Rochester, NY 14626
DEPARTMENT: Outpatient Mental Health
SCHEDULE: Monday-Friday, 8am-4:30pm, No weekend requirement
ATTRIBUTES
- Required Education: Minimum of a Bachelor’s degree in Health Science, Psychology, Social Work or the alike in a Human Service field. Masters preferred.
- Required License/Certifications: LCAT, LMFT, PhD, or Licensed Psychologist (i.e. LCSW, LMHC, LMSW, LCAT, LMFT, PhD, Licensed Psychologist required or credentialed in CASAC, CRC, CTRS. (Case Management Services excluded)
RESPONSIBILITIES
- Communicates appropriately with referral sources, collateral contacts significant to patient care, and quality monitoring committees or offices, as needed, and in a timely and complete manner.
- Monitors compliance with Department and State regulations and standards for patient care and documentation. Assists in the implementation and monitoring of program goals and objectives as well as patient care initiatives.
- Supports and monitors utilization of the electronic documentation system. Completes all required documentation in a timely and comprehensive manner, serving as role model for team members.
- Provides leadership and is responsible for the day-to-day functioning of the clinical team or program staff.
- Develops clinical programs in collaboration with Program and Department leadership, and evaluates success.
- Provides initial and ongoing assessments, and works with patients to develop and implement objective, measurable, and realistic treatment goals and aftercare objectives for discharge planning.
- Provides individual and group therapy, and monitors patient response to medications, in collaboration with the staff psychiatrist.
- Maintains a designated caseload, and/or a designated volume of clinical service (such as crisis intakes, no-show group facilitation, etc) and works to achieve a low percentage of failed appointments, guiding team members to do the same.
- Ensures that functional, efficient processes occur at the team level relative to patient referrals, admissions, treatment and discharges.
- Monitors the skills, interests, treatment efficacy, productivity, and schedules of team members in order to determine the assignment of patients and to ensure that the highest standards of patient care are practiced.
- Assists the Manager / Director or Psychiatrist with the interviewing, hiring, and training of new staff, and implements staff development and training initiatives.
PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
PAY RANGE: $70,500.00 - $100,000.00
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
