Position Summary: Responsible for providing services to both inpatients and outpatients, including assessment, treatment, planning, implementation and related documentation.

Position Requirements:

Education: Graduate of accredited School of Occupational Therapy.

Licensure: Licensed as an Occupational Therapist in the State of Illinois.

Certifications: Holds and maintains requirements for credential of “Registered” as issued by NBCOT.

Certified in CPR through the American Heart Association.
