Job Title: Comprehensive Perinatal Services Program (CPSP) Worker

Salary: $26- $31 hourly (DOE)

Openings: (1) Full-time

Location: Tustin, CA and Garden Grove, CA

Position Purpose: The Comprehensive Perinatal Services Program (CPSP) worker coordinates prenatal care for all patients in FTOC’s Prenatal Program. Provides patient-centered education and counseling; addresses patients’ unique needs by providing routine perinatal health education and counseling services per Comprehensive Perinatal Services Program (CPSP) requirements.

Core Duties and responsibilities, include but are not limited to:

  • Coordinates prenatal and postpartum care services for CPSP patients of Families Together of Orange County.
  • Provides patient-centered perinatal health education, counseling, and support in accordance with Comprehensive Perinatal Services Program requirements.
  • Conducts client orientations and completes required prenatal and postpartum assessments to identify patient needs, risks, and barriers to care.
  • Develops, updates, and monitors individualized care plans for patients enrolled in the Prenatal/CPSP Program.
  • Provides ongoing prenatal and postpartum health education services on an individual or group basis, including topics related to pregnancy, postpartum recovery, newborn care, breastfeeding, family planning, and overall wellness.
  • Provides nutrition education, assists patients with completing required nutritional assessments, including prenatal food frequency questionnaires, and monitors weight trends throughout pregnancy.
  • Screens patients for psychosocial needs, domestic violence, behavioral health concerns, social determinants of health, and other risk factors, and coordinates referrals as appropriate.
  • Facilitates access to medical, clinical, behavioral health, nutritional, social service, and community-based resources for patients, their children, and their families.
  • Coordinates with providers, Medical Assistants, clinic staff, and other internal departments to support comprehensive, coordinated, and timely care for perinatal patients.
  • Utilizes the Electronic Medical Record system to accurately and timely document assessments, care plans, education provided, referrals, patient appointments, newborn outcomes, and other required health information in accordance with FTOC documentation standards.
  • Follows all CPSP protocols, program requirements, workflows, and compliance expectations under the supervision and guidance of the Medical Director and/or Clinic Administrator.
  • Supports clinic operations and patient flow under the direction of the Clinic Manager, Clinic Administrator, or designated leadership.
  • Participates in monthly meetings, in-service trainings, off-site trainings, educational campaigns, and awareness activities to remain current on CPSP requirements, perinatal health trends, and community resources.
  • Assists with program reporting, narrative activity summaries, quality reviews, outcome tracking, quarterly reports, and other documents used to evaluate, analyze, and improve CPSP program activities.
  • Performs other related duties as assigned to support the Prenatal/CPSP Program, clinic operations, patient care coordination, and organizational needs

Requirements

Education, Qualifications, and Experience:

  • High school diploma or equivalent.
  • 2 years of prenatal experience required; 4+ years’ experience preferred.
  • CPSP certificate or completion of CPSP certification within 6 months of employment.
  • CPR certified required
  • Medical Assistant certificate required
  • Bilingual English/Spanish required.
  • EHR (eClinicalWorks) experience preferred
  • Ability to work with culturally diverse populations.
  • Ability to act independently and within a team.
  • Ability to establish work priorities to meet established deadlines.

Work Schedule:

  • Schedule for this role is Monday - Friday between 9 AM- 6 PM
  • Possibility for a Saturday or Sunday shift ( Saturday 8 AM -5 PM Sunday 8 AM -3 PM )
  • FTOC is an in-person organization first, and foremost. Employees are expected to be on-site for their scheduled shifts.
  • Hours of operation are Monday to Friday 8 a.m. to 8 p.m., however, employee schedules vary, depending on organizational, staffing, community, and patient needs. As such, FTOC may need to modify work schedules to meet such needs.
  • Holidays and weekends may be required depending on an employee’s department due to organizational, staffing, community, and patient needs as FTOC continues to grow and expand work days and hours.
  • Overtime may also occur due to organizational, staffing, community, and patient needs.

Work Location:

  • This role is set to float between the sites in Tustin, CA and Garden Grove, CA.
  • While employees may be assigned a primary site (Tustin, Garden Grove, Fountain Valley, Wellness Center, Mobile Units, Treehaven, Magnolia, Telehealth, etc.), employees may be required to work at another site based on organizational, staffing, community, and patient needs.
  • This role may be performed in an office, clinic, or community-based setting depending on organizational needs. The position may require prolonged periods of sitting, frequent computer use, travel to outreach events or partner sites, and the ability to communicate effectively in person, by phone, and in writing.

  • While performing the duties of this job, the employee may be required to drive to FTOC facilities as needed. The ability to sit for extended periods of time, and the ability to occasionally lift and/or move up to 25 pounds.
